颶風后的奧爾良，全城陷入癱瘓。當地的監獄中，所有獄警和囚犯已全部撤離，卻有一個囚徒被遺忘在這里。警官特倫斯•麥克多納（尼古拉斯•凱奇 Nicolas Cage 飾）和同事巡查牢房時發現了他，特倫斯出手相助。這個英勇舉動使他晉升為中尉，但他的脊柱也受到嚴重損傷，從此只能靠藥物緩解疼痛。不斷加劇的疼痛令各種藥物失去效力，特倫斯在這一過程中飽受折磨，他不得以求助毒品，身體和經濟狀況都接近崩潰邊緣，脾氣也越來越糟。一個英雄警察逐漸墮落。不久，城內發生一起滅門慘案，大毒梟達費特具有重大嫌疑。特倫斯受命調查此案，在這一過程中，他泯滅已久的良知逐漸復蘇……
