颶風(fēng)后的奧爾良，全城陷入癱瘓。當(dāng)?shù)氐谋O(jiān)獄中，所有獄警和囚犯已全部撤離，卻有一個囚徒被遺忘在這里。警官特倫斯•麥克多納（尼古拉斯•凱奇 Nicolas Cage 飾）和同事巡查牢房時發(fā)現(xiàn)了他，特倫斯出手相助。這個英勇舉動使他晉升為中尉，但他的脊柱也受到嚴(yán)重?fù)p傷，從此只能靠藥物緩解疼痛。不斷加劇的疼痛令各種藥物失去效力，特倫斯在這一過程中飽受折磨，他不得以求助毒品，身體和經(jīng)濟(jì)狀況都接近崩潰邊緣，脾氣也越來越糟。一個英雄警察逐漸墮落。不久，城內(nèi)發(fā)生一起滅門慘案，大毒梟達(dá)費(fèi)特具有重大嫌疑。特倫斯受命調(diào)查此案，在這一過程中，他泯滅已久的良知逐漸復(fù)蘇……
